The key to timely claim processing is to be as accurate as possible; please gather as much information from the accident scene as you can. Depending on the type of claim you’re reporting, different information will be needed. Please provide as much detail as possible in the form below.

• Make, model and VIN of insured vehicle involved
• Make, model and plate numbers of other vehicles involved
• Name, address, home/work/cell telephone number of witnesses
• Insurance information on the driver(s) and owner(s) of the other vehicles involved
